TRETABREVA COMB 110 / 50 MCG ( INDACATEROL / GLYCOPYRRONIUM ) POWDER FOR INHALATION IN HARD CAPSULES — 30 CAPSULES + INHALER

TRETABREVA COMB — Indacaterol 110 mcg / Glycopyrronium 50 mcg

Dosage form: Powder for inhalation in hard capsules • Pack: 30 capsules + 1 inhaler

Medical Description

Tretabreva Comb is a dual-action maintenance bronchodilator containing indacaterol and glycopyrronium. It is designed to relieve symptoms of chronic obstructive pulmonary disease (COPD) by relaxing airway muscles through two complementary mechanisms, helping improve airflow and reduce breathing difficulties.

Indacaterol is a long-acting beta-2 adrenergic agonist (LABA), while glycopyrronium is a long-acting muscarinic antagonist (LAMA). Together, they provide sustained bronchodilation that may improve lung function, reduce breathlessness, and support daily activities in adults with COPD.

Important: Tretabreva Comb is a maintenance treatment, not a rescue medicine for sudden breathing attacks.

Indications and Potential Benefits

Tretabreva Comb is indicated for long-term bronchodilator treatment of adults with COPD, subject to the locally approved product information.

  • Relief of breathlessness: Helps reduce shortness of breath associated with persistent airflow limitation.
  • Improved lung function: Relaxes airway muscles through complementary bronchodilator mechanisms.
  • Reduced respiratory symptoms: May help relieve wheezing, chest tightness, and breathing discomfort associated with COPD.
  • Improved exercise tolerance: Better airflow may help patients perform daily physical activities more comfortably.
  • Maintenance symptom control: Provides sustained bronchodilation when used regularly as prescribed.

Mechanism of Action

Tretabreva Comb combines two long-acting bronchodilators:

1. Indacaterol 110 mcg — LABA

Indacaterol selectively stimulates beta-2 adrenergic receptors in bronchial smooth muscle, activating pathways that relax the airway muscles and produce bronchodilation.

2. Glycopyrronium 50 mcg — LAMA

Glycopyrronium blocks muscarinic receptors, particularly M3 receptors, in bronchial smooth muscle. This inhibits acetylcholine-mediated bronchoconstriction and helps keep the airways open.

Combined effect: These complementary mechanisms help improve airflow and reduce COPD symptoms more effectively than either bronchodilator alone in many patients.

Dosage and Administration

  • Usual regimen for the corresponding 110/50 mcg formulation: Inhale the contents of one capsule once daily, at approximately the same time each day, unless the locally approved Tretabreva Comb leaflet specifies otherwise.
  • Route: Oral inhalation using the supplied capsule-based inhaler.
  • Administration: Place one capsule in the inhaler, pierce it using the device mechanism as instructed, and inhale the powder deeply through the mouthpiece according to the device instructions.
  • Important: The capsule is for inhalation only. Do not swallow the capsule.
  • Remove the empty capsule after use and follow the manufacturer's instructions for cleaning and storing the inhaler.

Do not exceed the prescribed dose. If a dose is missed, follow the package leaflet and do not take an extra dose to compensate.

Contraindications and Precautions

  • Known hypersensitivity to indacaterol, glycopyrronium, or any formulation ingredient.
  • Cardiovascular disease: Use cautiously in patients with coronary artery disease, arrhythmias, hypertension, or other significant cardiovascular conditions.
  • Glaucoma: Avoid accidental exposure of the inhaled powder to the eyes, as glycopyrronium may worsen narrow-angle glaucoma.
  • Urinary retention: Use cautiously in patients with an enlarged prostate, bladder-neck obstruction, or a history of urinary retention.
  • Respiratory conditions: Use caution in patients with asthma unless specifically prescribed under an appropriate treatment plan; this LABA/LAMA combination is not a substitute for inhaled corticosteroid-containing therapy when that is indicated.
  • Paradoxical bronchospasm: Stop use and seek immediate medical attention if breathing suddenly worsens after inhalation.

Possible Side Effects

Potential adverse effects include:

  • Cough or throat irritation.
  • Headache and dizziness.
  • Dry mouth.
  • Palpitations or increased heart rate.
  • Muscle cramps.
  • Constipation.
  • Urinary difficulty or retention.

Serious adverse effects requiring urgent assessment include severe allergic reactions, sudden worsening of breathing, chest pain, significant palpitations, or acute eye pain accompanied by blurred vision or halos.

The risk and frequency of adverse effects may vary according to the patient and the exact formulation.

Drug Interactions

Inform the treating physician or pharmacist about all medicines being used, particularly:

  • Other anticholinergic medicines: May increase the risk of dry mouth, urinary retention, and other anticholinergic adverse effects.
  • Other beta-2 agonists: Concurrent use may increase cardiovascular and other sympathomimetic effects.
  • Beta-blockers: May reduce the bronchodilator effect of indacaterol; use requires clinical assessment.
  • Medicines that prolong the QT interval or affect heart rhythm: May require additional caution in susceptible patients.
  • Potassium-depleting medicines: Certain diuretics and other medicines may increase the risk of low potassium, particularly in susceptible patients.

Do not add another long-acting bronchodilator without medical advice.

Storage

Store according to the manufacturer's instructions on the package leaflet. Keep capsules in their original blister packaging until use, protect them from moisture, and keep them out of reach of children. Use only the supplied or specifically approved inhaler, and do not swallow the capsules.
