METFORMIN EVAPHARMA XR 1000 MG ( METFORMIN HYDROCHLORIDE ) 30 EXTENDED-RELEASE TABLETS

METFORMIN EVAPHARMA XR 1000 MG

Metformin Hydrochloride | 30 Extended-Release Tablets

Medical Description

Metformin Evapharma XR 1000 mg contains metformin hydrochloride, an oral antidiabetic medication belonging to the biguanide class. It is formulated as an extended-release (XR) tablet, allowing gradual release of the active ingredient over time.

Metformin is primarily used to improve blood glucose control in patients with type 2 diabetes mellitus. It works mainly by reducing glucose production in the liver, decreasing intestinal glucose absorption, and improving insulin sensitivity in peripheral tissues, thereby helping the body utilize insulin more effectively.

The extended-release formulation is designed to provide prolonged drug release and may offer convenient once-daily administration, depending on the prescribed regimen.

Metformin is generally used alongside a balanced diet and regular physical activity and may be prescribed alone or in combination with other antidiabetic medications when additional glycemic control is required.

Strength

Metformin Hydrochloride

1000 mg

Active ingredient characteristics:

  • Metformin Hydrochloride: A biguanide antidiabetic agent that helps lower blood glucose levels without directly stimulating insulin secretion.
  • Extended-Release Technology: Provides gradual release of metformin, helping maintain therapeutic drug exposure over an extended period.

Indications and Uses

Metformin XR is primarily indicated for:

  1. Type 2 Diabetes Mellitus: Helps improve glycemic control in adults with type 2 diabetes.
  2. Fasting Blood Glucose Control: Reduces excessive hepatic glucose production, helping lower fasting blood glucose levels.
  3. Postprandial Glucose Management: Helps reduce glucose absorption from the gastrointestinal tract.
  4. Improved Insulin Sensitivity: Enhances the body's response to naturally produced insulin.
  5. Combination Diabetes Therapy: May be used alongside other antidiabetic medications, including insulin, when additional blood glucose control is needed.

Additional clinical use: Metformin may sometimes be prescribed off-label for selected patients with polycystic ovary syndrome (PCOS), particularly when metabolic abnormalities or insulin resistance are present.

Mechanism of Action

Metformin lowers blood glucose through several complementary mechanisms:

  • Reduction of Hepatic Glucose Production: Suppresses glucose production in the liver, particularly through inhibition of hepatic gluconeogenesis.
  • Improved Insulin Sensitivity: Enhances insulin-mediated glucose uptake and utilization in peripheral tissues, especially skeletal muscle.
  • Reduced Intestinal Glucose Absorption: Decreases the absorption of glucose from the gastrointestinal tract.
  • Metabolic Regulation: Improves overall glycemic control without directly stimulating pancreatic insulin secretion.

Extended-release mechanism: The XR formulation gradually releases metformin throughout the gastrointestinal tract, helping provide prolonged drug exposure and potentially improving gastrointestinal tolerability compared with immediate-release formulations.

Dosage and Administration

  • The dosage should be individualized according to blood glucose levels, treatment response, kidney function, and tolerability.
  • Metformin XR is generally administered once daily with the evening meal, according to the specific formulation's prescribing instructions.
  • Treatment is usually initiated at a lower dose and gradually increased to minimize gastrointestinal adverse effects.
  • The 1000 mg tablet should be swallowed whole with sufficient water.
  • Do not crush, chew, or split the extended-release tablet unless the specific manufacturer's instructions explicitly permit it.
  • If additional glycemic control is required, the physician may adjust the total daily dose or prescribe combination therapy.

Important: Metformin XR 1000 mg is a prescription medication. Patients should not independently change the dose or switch between immediate-release and extended-release formulations.

Contraindications

Metformin XR is contraindicated in patients with:

  • Severe renal impairment (eGFR below 30 mL/min/1.73 m²).
  • Known hypersensitivity to metformin or any formulation ingredient.
  • Acute or chronic metabolic acidosis, including diabetic ketoacidosis.
  • Conditions associated with severe tissue hypoxia or unstable acute illness that significantly increases the risk of lactic acidosis.
  • Severe dehydration or acute conditions that may cause significant deterioration in kidney function.

Possible Side Effects

The most common adverse effects are gastrointestinal and may improve as the body adjusts to treatment.

Common side effects:

  • Diarrhoea.
  • Nausea and vomiting.
  • Abdominal discomfort or pain.
  • Bloating and flatulence.
  • Indigestion.
  • Loss of appetite.
  • Altered or metallic taste.

Other possible effects:

  • Vitamin B12 deficiency, particularly with prolonged use.
  • Mild reduction in body weight in some patients.
  • Skin rash or allergic reactions.
►Serious but rare complication

Lactic Acidosis: A rare but potentially life-threatening complication associated with metformin accumulation, particularly in patients with severe kidney impairment or conditions causing tissue hypoxia.

Warning symptoms may include unusual muscle pain, severe weakness, difficulty breathing, abdominal discomfort, hypothermia, and unusual drowsiness. Immediate medical attention is required if lactic acidosis is suspected.

Precautions and Warnings

  • Renal Function: Assess kidney function before starting treatment and periodically throughout therapy. Metformin is contraindicated when eGFR is below 30 mL/min/1.73 m².
  • Vitamin B12 Deficiency: Long-term metformin use may reduce vitamin B12 absorption. Periodic monitoring is recommended, particularly in patients with anaemia or peripheral neuropathy.
  • Lactic Acidosis: Use cautiously in patients with conditions that may increase the risk of metformin accumulation or impaired tissue oxygenation.
  • Alcohol Consumption: Excessive alcohol intake should be avoided because it may increase the risk of lactic acidosis.
  • Iodinated Contrast Procedures: Metformin may need to be temporarily discontinued before or at the time of certain contrast-enhanced imaging procedures, depending on renal function and clinical circumstances. Kidney function should be reassessed before restarting.
  • Surgery and Acute Illness: Temporary interruption may be necessary during certain surgical procedures, severe infections, dehydration, or other conditions associated with reduced food and fluid intake or impaired kidney function.
  • Combination Therapy: When used with insulin or sulfonylureas, the risk of hypoglycaemia may increase due to the accompanying medication.
  • Pregnancy and Breastfeeding: Treatment decisions should be made by the treating physician after evaluating the benefits and potential risks.

Storage Conditions

  • Store at room temperature according to the manufacturer's instructions.
  • Protect from excessive heat and moisture.
  • Keep the tablets in their original packaging.
  • Keep out of reach of children.
  • Do not use after the expiry date printed on the package.
