Medical Description

Blokatens HCT is a fixed-dose combination of three antihypertensive medicines: amlodipine, a calcium-channel blocker; valsartan, an angiotensin II receptor blocker (ARB); and hydrochlorothiazide (HCTZ), a thiazide diuretic.

The three medicines lower blood pressure through complementary mechanisms. This combination may be prescribed for adults whose blood pressure is not adequately controlled with two antihypertensive agents or when treatment with three complementary classes is clinically appropriate. Combination therapy can also simplify treatment by providing multiple medicines in a single tablet.

How It Works

Amlodipine:
Amlodipine blocks L-type calcium channels in vascular smooth muscle, producing relaxation and widening of peripheral arteries. This reduces systemic vascular resistance and consequently lowers blood pressure.

Valsartan:
Valsartan blocks the angiotensin II type 1 (AT₁) receptor. By inhibiting the effects of angiotensin II, it reduces vasoconstriction and aldosterone-mediated sodium and water retention, contributing to lower blood pressure.

Hydrochlorothiazide:
Hydrochlorothiazide inhibits sodium and chloride reabsorption in the distal renal tubule. This increases the excretion of sodium and water, reducing extracellular fluid volume and contributing to blood-pressure reduction.

Together, these complementary mechanisms can provide greater blood-pressure reduction than some two-drug combinations. Clinical evidence has demonstrated the effectiveness of triple therapy containing amlodipine, valsartan, and hydrochlorothiazide in patients with moderate-to-severe hypertension.

Therapeutic Use

Blokatens HCT is indicated for the treatment of hypertension (high blood pressure).

Lowering persistently elevated blood pressure reduces the risk of cardiovascular complications, including stroke, myocardial infarction, heart failure, and other cardiovascular events.

The combination is particularly useful when adequate blood-pressure control cannot be achieved with two antihypertensive classes or when a patient has already been individually titrated to the three components. Official labeling describes the triple combination as add-on or replacement therapy for patients inadequately controlled on two of the relevant antihypertensive classes.

Advantages of the Combination

Blokatens HCT provides three complementary antihypertensive mechanisms in a single tablet:

• Amlodipine: arterial vasodilation.
• Valsartan: inhibition of angiotensin II-mediated vasoconstriction.
• Hydrochlorothiazide: increased renal sodium and water excretion.

Dosage & Administration

The dose and strength should be individualized by the prescribing physician according to the patient's blood-pressure response, previous antihypertensive treatment, kidney function, electrolyte status, and tolerance.

For approved amlodipine/valsartan/hydrochlorothiazide products, administration is generally once daily, with the specific strength selected according to the individually titrated components.

Take the tablet exactly as prescribed, preferably at the same time each day. Do not change the dose or discontinue treatment without medical advice.

Because hydrochlorothiazide increases urine production, some patients may prefer taking the medicine earlier in the day, depending on their physician's instructions.

Important Warnings & Precautions

1. Pregnancy

Valsartan can cause fetal injury or death when used during pregnancy, particularly during the second and third trimesters. If pregnancy is detected, treatment should be discontinued as soon as possible and the patient should contact her healthcare professional.

Blokatens HCT should therefore not be used during pregnancy unless specifically directed by a qualified physician, and women who are pregnant or planning pregnancy should inform their healthcare professional.

2. Low Blood Pressure and Dizziness

Because all three components lower blood pressure, excessive hypotension may occur, particularly when treatment is initiated, doses are increased, or the patient is dehydrated.

Patients may experience:

• Dizziness
• Light-headedness
• Weakness
• Fainting

Patients should rise slowly from sitting or lying positions and seek medical advice if significant symptoms occur.

3. Electrolyte Disturbances

Hydrochlorothiazide can alter electrolyte levels, including sodium, potassium, and magnesium. Monitoring may be necessary, particularly in patients receiving prolonged treatment or those at increased risk of electrolyte abnormalities.

Symptoms such as unusual weakness, muscle cramps, palpitations, confusion, or severe fatigue should be medically evaluated.

4. Kidney Function

Valsartan and hydrochlorothiazide require consideration of renal function. Patients with kidney disease, dehydration, or conditions that may impair renal perfusion may require closer monitoring of serum creatinine and electrolytes.

5. Liver Disease

Patients with significant hepatic impairment should inform their physician before using the medicine because dose selection and monitoring may require special consideration.

6. Angioedema

Valsartan, like other medicines affecting the renin-angiotensin system, can rarely cause angioedema, characterized by swelling of the face, lips, tongue, throat, or other tissues.

Difficulty breathing or swallowing requires urgent medical attention.

Possible Side Effects

Potential adverse effects may include:

Common or relatively frequent effects:

• Dizziness
• Headache
• Fatigue
• Swelling of the ankles or feet (peripheral edema)
• Increased urination
• Flushing
• Nausea or gastrointestinal discomfort

Potential clinically important effects:

• Excessive reduction in blood pressure
• Electrolyte disturbances
• Changes in kidney function
• Increased blood glucose or uric acid
• Palpitations
• Hypersensitivity reactions
• Rare angioedema

Amlodipine is particularly associated with peripheral edema, while hydrochlorothiazide may contribute to electrolyte and metabolic abnormalities.

Contraindications & Important Situations

Blokatens HCT should not be used in patients with known hypersensitivity to amlodipine, valsartan, hydrochlorothiazide, or any component of the formulation.

Particular medical supervision is required in patients with:

• Significant kidney disease.
• Significant liver disease.
• Severe dehydration.
• Electrolyte abnormalities.
• Low blood pressure.
• History of angioedema associated with renin-angiotensin-system blockers.
• Gout or elevated uric acid.
• Diabetes mellitus.
• Systemic lupus erythematosus.
• Conditions associated with increased risk of electrolyte disturbances.

Drug Interactions & Treatment Considerations

Patients should tell their physician or pharmacist about all prescription medicines, over-the-counter medicines, supplements, and herbal products they use.

Important interactions may occur with:

• Other antihypertensive medicines.
• Diuretics.
• Potassium supplements or potassium-containing salt substitutes.
• Medicines that increase serum potassium.
• Non-steroidal anti-inflammatory drugs (NSAIDs).
• Lithium.
• Certain medicines affecting kidney function.
• Other medicines that may lower blood pressure.

Particular caution is required when valsartan is combined with medicines that increase potassium levels or when NSAIDs are used in patients with impaired kidney function.

Monitoring During Treatment

Depending on the patient's medical condition, the healthcare professional may monitor:

• Blood pressure.
• Kidney function.
• Serum potassium and sodium.
• Other electrolytes.
• Blood glucose in susceptible patients.
• Uric acid in patients at risk of gout.
• Symptoms of excessive hypotension or dehydration.

Lifestyle measures remain an important part of hypertension management. Current American Heart Association recommendations emphasize healthy weight management, a heart-healthy diet such as DASH, reduced sodium intake, regular physical activity, and appropriate alcohol reduction or avoidance.
