Medical Description

CHARTOREG CO 5/40 mg contains a fixed-dose combination of amlodipine 5 mg and telmisartan 40 mg, two antihypertensive medicines with complementary mechanisms of action.

Amlodipine is a dihydropyridine calcium-channel blocker that relaxes vascular smooth muscle and produces arterial vasodilation, thereby reducing peripheral vascular resistance and lowering blood pressure.

Telmisartan is an angiotensin II receptor blocker (ARB) that selectively blocks the AT₁ receptors activated by angiotensin II. This prevents angiotensin II-mediated vasoconstriction and promotes relaxation of blood vessels.

Together, the two medicines provide complementary blood-pressure-lowering effects and are used when treatment with a single antihypertensive agent does not provide adequate control or when combination therapy is clinically appropriate.

Indication

CHARTOREG CO 5/40 mg is indicated for the treatment of hypertension (high blood pressure) in adults.

The telmisartan/amlodipine combination may be used in patients whose blood pressure is not adequately controlled with telmisartan or amlodipine alone, or as initial combination therapy in selected patients who are likely to require more than one antihypertensive medicine to achieve their blood-pressure goal.

Effective treatment of hypertension helps reduce the risk of cardiovascular complications associated with persistently elevated blood pressure, including stroke and myocardial infarction.

How It Works

Amlodipine 5 mg

  • Blocks L-type calcium channels in vascular smooth muscle.
  • Promotes relaxation of arterial blood vessels.
  • Reduces peripheral vascular resistance.
  • Helps lower blood pressure.

Telmisartan 40 mg

  • Selectively blocks angiotensin II from binding to AT₁ receptors.
  • Reduces angiotensin II-induced vasoconstriction.
  • Promotes vasodilation and lowers blood pressure.

The combination targets two different physiological pathways involved in blood-pressure regulation.

Dosage & Administration

  • The usual recommended dose of this 5 mg/40 mg combination is one tablet once daily, when prescribed at this strength.
  • The dose should be individualized according to the patient's blood-pressure response and tolerability.
  • Telmisartan/amlodipine combinations may be titrated when additional blood-pressure reduction is required; the maximum recommended combination dose in the referenced prescribing information is 80 mg telmisartan/10 mg amlodipine once daily.
  • The medicine may be taken with or without food.
  • Take the tablet at approximately the same time each day and follow the dose prescribed by your physician.

Important Precautions

Inform your physician before taking CHARTOREG CO if you have:

  • Kidney disease or impaired kidney function
  • Liver disease
  • Low blood pressure or dehydration
  • A history of angioedema or serious allergic reaction to an ARB or calcium-channel blocker
  • Conditions that may affect blood potassium levels

Telmisartan may cause increased serum potassium and changes in kidney function in susceptible patients. Appropriate monitoring of blood pressure, kidney function and electrolytes may be required depending on the patient's clinical condition.

Avoid using potassium supplements or potassium-containing salt substitutes without medical advice.

Concurrent use of telmisartan with other medicines that block the renin-angiotensin system, such as ACE inhibitors or aliskiren, can increase the risk of hypotension, hyperkalemia and impaired kidney function and should generally be avoided unless specifically directed and monitored by a physician.

Possible Side Effects

Possible adverse effects may include:

  • Dizziness or lightheadedness
  • Headache
  • Peripheral edema (swelling of the ankles or feet)
  • Flushing
  • Low blood pressure
  • Palpitations
  • Diarrhea
  • Changes in kidney function or blood potassium levels

Peripheral edema is a recognized dose-related adverse effect of amlodipine.

Seek medical attention promptly if symptoms suggest a serious allergic reaction or angioedema, such as swelling of the face, lips, tongue or throat, particularly if accompanied by difficulty breathing.

Pregnancy Warning

CHARTOREG CO should not be used during pregnancy.

Telmisartan acts directly on the renin-angiotensin system and can cause serious fetal injury or death, particularly during the second and third trimesters. If pregnancy is detected, contact a healthcare professional immediately for alternative treatment.

Missed Dose

If you miss a dose, take it when you remember unless it is almost time for your next dose. Do not take two tablets at the same time to compensate for a missed dose.

Lifestyle Measures

For effective long-term blood-pressure management, medication should be accompanied when appropriate by lifestyle measures such as:

  • Reducing dietary salt/sodium intake
  • Maintaining a healthy body weight
  • Regular physical activity
  • Following a balanced diet
  • Avoiding tobacco
  • Limiting alcohol consumption

Storage

Keep the tablets in their original packaging and protected from moisture and light, following the storage conditions stated on the product package. Telmisartan/amlodipine tablets are moisture-sensitive.
