Finasteride — Topical/Cutaneous Use

Finasteride is a 5-alpha-reductase inhibitor that reduces the conversion of testosterone to dihydrotestosterone (DHT). DHT contributes to the miniaturization of susceptible scalp hair follicles in androgenetic alopecia (male-pattern hair loss). By reducing DHT activity around the hair follicle, finasteride can help slow hair loss and promote maintenance and regrowth of scalp hair.

Topical Finasteride

Topical/cutaneous finasteride is applied directly to the scalp with the aim of producing a local anti-androgenic effect while limiting systemic exposure compared with oral administration. Clinical studies have demonstrated that topical finasteride can reduce scalp DHT and improve hair counts in men with androgenetic alopecia. A Phase III randomized controlled trial of 0.25% topical finasteride spray found significantly improved hair counts compared with placebo, with substantially lower systemic exposure than oral finasteride.

Evidence from systematic reviews suggests that topical finasteride is a promising treatment option for androgenetic alopecia, although the available evidence is less extensive than that for oral finasteride, and optimal concentration, dose, frequency, and long-term safety depend on the specific formulation.

How It Is Used

Topical finasteride should be applied only according to the prescribed product's concentration, dose, and application instructions. The treatment generally requires consistent, long-term use, as improvement in androgenetic alopecia is gradual and discontinuation may result in loss of the treatment benefit.

Important: The concentration and number of sprays/drops cannot be standardized across all topical finasteride products because compounded and commercially prepared formulations differ. Therefore, the product-specific instructions or physician's prescription should always take precedence.

Possible Side Effects & Precautions

Topical administration may cause local scalp reactions, including irritation, redness, dryness, scaling, stinging, or burning. Although systemic exposure is generally lower than with oral finasteride, systemic absorption can still occur, so systemic adverse effects cannot be assumed to be completely eliminated.

Possible systemic effects associated with finasteride include sexual adverse effects such as decreased libido, erectile dysfunction, or ejaculation disorders. Mood-related adverse effects have also been reported with finasteride therapy.

Pregnancy & Accidental Exposure

Finasteride is contraindicated in pregnancy because exposure may cause abnormalities in the development of the external genitalia of a male fetus. Pregnant women or women who may become pregnant should avoid contact with topical finasteride products. Particular care is required with topical preparations because accidental transfer to another person is possible. The FDA has specifically highlighted the potential for inadvertent exposure from compounded topical finasteride.

The FDA also advises that pregnant women or women who may become pregnant should not handle crushed or broken finasteride tablets, because finasteride can be absorbed through the skin.

Important Clinical Note

Topical finasteride should not be considered automatically equivalent to oral finasteride. Although clinical evidence is encouraging, topical products differ considerably in formulation and dosing, and regulatory approval varies by country and product. The FDA has warned about risks associated with compounded topical finasteride products.
