Drug Class

Selective Cyclooxygenase-2 (COX-2) Inhibitor Nonsteroidal Anti-inflammatory Drug (NSAID)

Overview

Celecoxib is a selective COX-2 inhibitor NSAID indicated for the treatment of pain and inflammatory conditions. By selectively inhibiting the cyclooxygenase-2 enzyme responsible for prostaglandin synthesis at sites of inflammation, celecoxib provides effective analgesic, anti-inflammatory, and antipyretic effects while generally causing fewer upper gastrointestinal ulcers than non-selective NSAIDs. However, like all NSAIDs, celecoxib carries important cardiovascular, gastrointestinal, and renal safety considerations and should be prescribed at the lowest effective dose for the shortest duration consistent with treatment goals.

Mechanism of Action

Celecoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme while having minimal inhibition of COX-1 at therapeutic doses.

This results in:

  • Reduced prostaglandin production
  • Decreased inflammation
  • Relief of pain
  • Reduction of swelling
  • Lower fever

Because gastric mucosal protection is largely mediated by COX-1, celecoxib generally produces fewer endoscopically detected gastric ulcers than traditional NSAIDs, although clinically significant gastrointestinal bleeding can still occur.

Approved Therapeutic Indications

Celecoxib is indicated for:

  • Osteoarthritis (OA)
  • Rheumatoid arthritis (RA)
  • Juvenile idiopathic arthritis (JIA) (selected pediatric patients)
  • Ankylosing spondylitis
  • Acute pain
  • Primary dysmenorrhea (menstrual pain)

It is also used according to specialist recommendations in selected inflammatory musculoskeletal disorders.

Clinical Benefits

Evidence from randomized controlled trials and systematic reviews demonstrates that celecoxib:

  • Effectively reduces pain and inflammation
  • Improves physical function in osteoarthritis
  • Relieves symptoms of rheumatoid arthritis
  • Improves mobility
  • Reduces morning stiffness
  • Provides postoperative and acute pain relief
  • Produces fewer upper gastrointestinal ulcers than many traditional NSAIDs in appropriate patients

The large PRECISION trial (published in NEJM) found celecoxib to have cardiovascular safety comparable to ibuprofen and naproxen when used at approved doses in patients with arthritis requiring long-term NSAID therapy.

Dosage

Dosage depends on the treated condition.

Osteoarthritis

  • 200 mg/day
    • 200 mg once daily
    • OR 100 mg twice daily

Rheumatoid Arthritis

  • 100–200 mg twice daily

Ankylosing Spondylitis

  • 200 mg/day
  • May increase to 400 mg/day if needed.

Acute Pain

  • Initial dose:
    • 400 mg
  • Additional 200 mg on Day 1 if needed
  • Then:
    • 200 mg twice daily as required

Primary Dysmenorrhea

Same regimen as acute pain.

Administration

  • Take with or without food.
  • Swallow capsules whole.
  • May be taken with food if stomach upset occurs.
  • Maintain adequate hydration.
  • Do not exceed the prescribed dose.
  • Use the shortest treatment duration necessary.

Common Side Effects

Most adverse effects are mild.

Common reactions include:

  • Dyspepsia
  • Abdominal pain
  • Diarrhea
  • Flatulence
  • Nausea
  • Headache
  • Dizziness
  • Upper respiratory tract infections
  • Peripheral edema

Serious Adverse Effects

Although uncommon, serious adverse events include:

Cardiovascular

  • Myocardial infarction
  • Stroke
  • Hypertension
  • Heart failure exacerbation

Gastrointestinal

  • Gastric ulcer
  • Gastrointestinal bleeding
  • Perforation

Renal

  • Acute kidney injury
  • Fluid retention
  • Hyperkalemia

Hepatic

  • Elevated liver enzymes
  • Hepatitis
  • Rare liver failure

Dermatologic

  • Stevens–Johnson syndrome
  • Toxic epidermal necrolysis
  • DRESS syndrome

Allergic

  • Anaphylaxis
  • Angioedema

Contraindications

Celecoxib should not be used in patients with:

  • Hypersensitivity to celecoxib
  • Allergy to sulfonamides
  • Aspirin- or NSAID-induced asthma or urticaria
  • Perioperative pain after coronary artery bypass graft (CABG) surgery
  • Active gastrointestinal bleeding
  • Severe hepatic impairment

Warnings and Precautions

Use with caution in patients with:

  • Cardiovascular disease
  • Hypertension
  • Chronic kidney disease
  • Liver disease
  • History of peptic ulcer disease
  • Gastrointestinal bleeding
  • Elderly patients
  • Patients taking anticoagulants
  • Patients receiving corticosteroids
  • Dehydration

Monitor:

  • Blood pressure
  • Renal function
  • Liver function
  • Gastrointestinal symptoms
  • Signs of edema or heart failure

Drug Interactions

Important interactions include:

  • Warfarin and other anticoagulants
  • Aspirin
  • Other NSAIDs
  • Corticosteroids
  • ACE inhibitors
  • ARBs
  • Diuretics
  • Lithium
  • Methotrexate
  • Cyclosporine
  • Tacrolimus
  • Fluconazole (may increase celecoxib exposure)

Pregnancy

  • Avoid use at 20 weeks' gestation or later unless specifically advised by a healthcare professional due to the risk of fetal renal dysfunction and oligohydramnios.
  • Contraindicated from approximately 30 weeks' gestation onward because of the risk of premature closure of the fetal ductus arteriosus.
  • Use during early pregnancy only if the potential benefit outweighs the risk.

Breastfeeding

Celecoxib appears in breast milk in small amounts. Limited evidence suggests low infant exposure, but treatment during breastfeeding should be based on an individualized risk–benefit assessment by a healthcare professional.

Pediatric Use

Approved for:

  • Juvenile idiopathic arthritis (JIA)
  • Children 2 years of age and older meeting appropriate weight criteria

Safety and efficacy have not been established for other pediatric indications.

Geriatric Use

Older adults have an increased risk of:

  • Gastrointestinal bleeding
  • Renal impairment
  • Cardiovascular events

Careful dose selection and clinical monitoring are recommended.

Storage

  • Store at 20–25°C (68–77°F).
  • Protect from excessive heat and moisture.
  • Keep in the original package.
  • Keep out of reach of children.

Key Counseling Points

Patients should:

  • Take exactly as prescribed.
  • Avoid taking additional NSAIDs unless instructed by their healthcare provider.
  • Seek immediate medical attention for chest pain, sudden weakness, slurred speech, black stools, vomiting blood, severe abdominal pain, swelling, or difficulty breathing.
  • Inform their healthcare provider of all medications they are taking, especially blood thinners, corticosteroids, and antihypertensive drugs.
  • Attend regular follow-up appointments if long-term therapy is required.
