Glimepiride / Metformin hydrochloride

Glimepiride/Metformin HCl is a fixed-dose oral antidiabetic combination used in the management of type 2 diabetes mellitus (T2DM). It combines:

  • Glimepiride (a sulfonylurea insulin secretagogue)
  • Metformin hydrochloride (a biguanide insulin sensitizer)

Mechanism of Action

This combination provides dual complementary glucose-lowering effects:

1. Glimepiride

  • Stimulates insulin secretion from pancreatic β-cells
  • Works by blocking ATP-sensitive potassium channels, leading to calcium influx and insulin release
  • Effective only when some pancreatic insulin function is present

2. Metformin

  • Reduces hepatic glucose production (gluconeogenesis)
  • Improves peripheral insulin sensitivity (muscle and adipose tissue)
  • Enhances glucose uptake and utilization

Combined Effect

  • Addresses both:
    • Insulin deficiency (via glimepiride)
    • Insulin resistance (via metformin)
  • Results in improved overall glycemic control

Indications

  • Management of type 2 diabetes mellitus when:
