Carboplatin is a second-generation platinum-based chemotherapy drug. It was developed as a less toxic analog to cisplatin, offering a reduced risk of kidney damage and nausea while remaining highly effective against various solid tumors.
Medical Description and Mechanism
- Classification: It is an alkylating agent and an organoplatinum antineoplastic.
- Mechanism of Action: Carboplatin acts as a molecular "handcuff" for DNA. It undergoes intracellular activation to form reactive platinum complexes that bind to nuclear DNA, creating interstrand and intrastand cross-links. These cross-links "glue" the DNA strands together, preventing the cell from unzipping its DNA for replication or transcription. This triggers apoptosis (programmed cell death) in rapidly dividing cancer cells.
- Pharmacokinetics: Administered via intravenous (IV) infusion. It is eliminated primarily through the kidneys, with roughly 70% of the dose excreted in the urine within 24 hours.
- Dosing: Unlike many drugs, carboplatin doses are often calculated using the Calvert Formula, which accounts for a patient's specific kidney function (glomerular filtration rate or GFR) to ensure an optimal target "Area Under the Curve" (AUC).
Therapeutic Indications
Carboplatin is used both as a primary treatment and as palliative care for:
Ovarian Cancer: Specifically advanced epithelial ovarian carcinoma, often in combination with cyclophosphamide or paclitaxel.
Lung Cancer: Both small cell (SCLC) and non-small cell lung cancer (NSCLC).
Other Malignancies: Head and neck cancer, brain tumors, neuroblastoma, and triple-negative breast cancer.
Transplant Prep: Sometimes used in conditioning regimens for stem cell or bone marrow transplants.
Clinical Warnings & Safety
- Myelosuppression: This is the "dose-limiting" toxicity. It can cause severe drops in white blood cells (neutropenia), red blood cells (anemia), and platelets (thrombocytopenia), significantly increasing the risk of life-threatening infections and bleeding.
- Anaphylaxis: Severe allergic reactions can occur within minutes of administration, particularly in patients who have received six or more previous doses of platinum therapy.
- Ototoxicity & Neurotoxicity: While less common than with cisplatin, it can still cause ringing in the ears (tinnitus), hearing loss, and peripheral neuropathy (numbness/tingling in hands and feet).
- Kidney & Liver Function: Requires regular monitoring of GFR and liver enzymes. It is contraindicated in patients with severe renal impairment (typically GFR < 20-30 mL/min).
