Pioglitazone is an oral anti-diabetic agent in the thiazolidinedione (TZD) class used primarily as an adjunct to diet and exercise for managing type 2 diabetes mellitus.

Mechanism of Action

Insulin Sensitization: Pioglitazone is a potent and selective agonist for the peroxisome proliferator-activated receptor-gamma (PPAR), located in the nucleus of cells in adipose tissue, skeletal muscle, and the liver.
Gene Transcription: Activation of these receptors modulates the transcription of genes involved in glucose and lipid metabolism, increasing the expression of glucose transporters like GLUT4.
Physiological Impact: It reduces peripheral insulin resistance and hepatic glucose production, thereby lowering blood sugar levels.
Secondary Effect: It also has weak activity at the PPAR receptor, which contributes to its ability to lower triglyceride levels.

Indications and Usage

Primary Use: Indicated for adults with type 2 diabetes as monotherapy or in combination with metformin, sulfonylureas, or insulin.
Administration: Typically taken as a 15 mg, 30 mg, or 45 mg tablet once daily, with or without food.
Limitations: It is not used for type 1 diabetes or diabetic ketoacidosis, as it requires the presence of endogenous insulin to be effective.
