Pioglitazone is an oral anti-diabetic agent in the thiazolidinedione (TZD) class used primarily as an adjunct to diet and exercise for managing type 2 diabetes mellitus.

Mechanism of Action
  • Insulin Sensitization: Pioglitazone is a potent and selective agonist for the peroxisome proliferator-activated receptor-gamma (PPAR), located in the nucleus of cells in adipose tissue, skeletal muscle, and the liver.
  • Gene Transcription: Activation of these receptors modulates the transcription of genes involved in glucose and lipid metabolism, increasing the expression of glucose transporters like GLUT4.
  • Physiological Impact: It reduces peripheral insulin resistance and hepatic glucose production, thereby lowering blood sugar levels.
  • Secondary Effect: It also has weak activity at the PPAR receptor, which contributes to its ability to lower triglyceride levels. 

Indications and Usage
  • Primary Use: Indicated for adults with type 2 diabetes as monotherapy or in combination with metformin, sulfonylureas, or insulin.
  • Administration: Typically taken as a 15 mg, 30 mg, or 45 mg tablet once daily, with or without food.
  • Limitations: It is not used for type 1 diabetes or diabetic ketoacidosis, as it requires the presence of endogenous insulin to be effective.

Contraindications Symptomatic heart failure (NYHA Class III or IV), active bladder cancer, and active liver disease.
Adverse Effects Most common include weight gain, peripheral edema (fluid retention), headache, and upper respiratory tract infections.
Serious Risks Small increased risk of bladder cancer with long-term use and increased risk of bone fractures, particularly in women.
Monitoring Liver function tests should be performed prior to starting treatment. Patients must be monitored for sudden weight gain or shortness of breath.
