Medical Description

CHARTOREG CO 5/80 mg contains a fixed-dose combination of amlodipine 5 mg and telmisartan 80 mg, two antihypertensive medicines that lower blood pressure through complementary mechanisms.

Amlodipine is a dihydropyridine calcium-channel blocker (CCB). It reduces calcium entry into vascular smooth muscle, producing arterial vasodilation and a reduction in peripheral vascular resistance, thereby lowering blood pressure.

Telmisartan is an angiotensin II receptor blocker (ARB). It selectively blocks the AT₁ receptor, preventing angiotensin II from producing vasoconstriction and other blood-pressure-raising effects. This promotes vascular relaxation and lowers blood pressure.

The 80 mg telmisartan/5 mg amlodipine strength is an established fixed-dose combination. The prescribing information identifies 80/5 mg as an available strength and as an option for patients requiring a greater initial blood-pressure reduction.

Indication

CHARTOREG CO 5/80 mg is indicated for the treatment of hypertension (high blood pressure) in adults.

The combination may be used when blood pressure is not adequately controlled with amlodipine or telmisartan alone, or when a patient is likely to require more than one antihypertensive medicine to achieve the desired blood-pressure control.

Effective treatment of hypertension reduces the risk of cardiovascular complications, particularly stroke and myocardial infarction.

Dosage & Administration

  • CHARTOREG CO 5/80 mg should be taken exactly as prescribed by the treating physician.
  • Telmisartan/amlodipine combinations are generally administered once daily.
  • The referenced prescribing information lists 80 mg/5 mg once daily as an available combination strength and states that the dose may be increased after at least 2 weeks when needed, up to a maximum of 80 mg telmisartan/10 mg amlodipine once daily.
  • It may be taken with or without food.

Important Precautions

Patients should consult their healthcare professional before using potassium supplements or potassium-containing salt substitutes.

Concomitant use of aliskiren with telmisartan is contraindicated in patients with diabetes because of the increased risk associated with dual renin-angiotensin system blockade.

Possible Side Effects

Possible adverse effects may include:

  • Dizziness or lightheadedness
  • Peripheral edema (swelling of the ankles or feet)
  • Headache
  • Flushing
  • Palpitations
  • Low blood pressure
  • Diarrhea
  • Back pain
  • Changes in kidney function
  • Increased blood potassium levels

Peripheral edema is a recognized adverse effect of amlodipine and can be dose-related.

Seek urgent medical attention if severe swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, fainting, or severe chest pain occurs, as these may indicate a serious adverse reaction.

Pregnancy Warning

CHARTOREG CO should not be used during pregnancy.

Telmisartan affects the renin-angiotensin system and can cause serious fetal injury or death, particularly during the later stages of pregnancy. If pregnancy occurs while taking CHARTOREG CO, contact a healthcare professional immediately.

Missed Dose

If you miss a dose, take it when you remember unless it is almost time for the next scheduled dose. Do not take two tablets at the same time to compensate for a missed dose.

Lifestyle Measures

Alongside prescribed treatment, lifestyle measures can contribute to blood-pressure control, including:

  • Reducing dietary salt/sodium intake
  • Maintaining a healthy body weight
  • Regular physical activity
  • Following a balanced diet
  • Avoiding tobacco
  • Limiting alcohol consumption

WHO recommends lifestyle modification together with appropriate pharmacological treatment for hypertension.

Storage

Keep CHARTOREG CO in its original packaging, protected from moisture and light, and follow the storage conditions stated on the product packaging. Telmisartan/amlodipine tablets should be protected from moisture and kept in their blister packaging until use where applicable.
