Medical Description

MICONAZ 2% topical cream contains miconazole nitrate 2%, a topical imidazole antifungal medicine used to treat susceptible fungal infections of the skin. Miconazole works by interfering with fungal cell-membrane synthesis, thereby inhibiting fungal growth and contributing to elimination of the infection.

Miconazole 2% topical preparations are used for common superficial fungal infections including tinea pedis (athlete's foot), tinea cruris (jock itch), tinea corporis (ringworm), cutaneous candidiasis, and pityriasis versicolor, depending on the specific product and diagnosis.

Composition

Each 100 g of cream contains:

  • Miconazole nitrate 2 g
    • Equivalent to Miconazole 2% w/w

Pack size: 20 g topical cream.

Indications

MICONAZ 2% may be used for the topical treatment of susceptible superficial fungal infections, including:

  • Tinea pedis — athlete's foot
  • Tinea cruris — jock itch
  • Tinea corporis — ringworm
  • Cutaneous candidiasis
  • Pityriasis (tinea) versicolor
  • Other susceptible superficial dermatophyte or yeast infections, when appropriately diagnosed.

The exact indication should be based on the clinical diagnosis because not every topical miconazole product is labeled for every fungal condition.

Mechanism of Action

Miconazole is an imidazole antifungal agent. It interferes with fungal ergosterol synthesis, an essential component of the fungal cell membrane.

This disruption alters fungal membrane integrity and function, ultimately inhibiting fungal growth and contributing to the control of the infection.

Clinical Benefits

When used appropriately for a susceptible superficial fungal infection, MICONAZ 2% may:

  • Treat the underlying fungal infection.
  • Inhibit fungal growth and replication.
  • Help relieve associated itching, scaling, burning, redness and discomfort.
  • Support restoration of normal skin when combined with appropriate hygiene and skin-care measures.

Dosage & Administration

For most superficial fungal skin infections:

  • Clean and thoroughly dry the affected area before application.
  • Apply a thin layer of cream to the affected skin, usually twice daily (morning and evening).
  • Gently spread the cream over the affected area and, where appropriate, the surrounding skin.
  • Wash hands before and after application unless the hands themselves are being treated.
  • Continue treatment for the recommended duration even if symptoms begin to improve, unless otherwise instructed by a healthcare professional.

Typical treatment durations vary according to the infection. WHO guidance describes approximately 2–4 weeks for cutaneous candidiasis and around 2–3 weeks for dermatophyte infections, while individual product labeling may specify different durations.

For OTC-labeled miconazole products, treatment is commonly recommended for 4 weeks for athlete's foot or ringworm and 2 weeks for jock itch.

Important Application Advice

For athlete's foot:

  • Carefully dry the spaces between the toes before application.
  • Keep feet clean and dry.
  • Wear well-fitting, ventilated footwear.
  • Change socks regularly to help reduce moisture and reinfection.

Avoid sharing towels, clothing, footwear or other personal items that may contribute to transmission or reinfection.

Warnings & Precautions

For external use only.

  • Avoid contact with the eyes and mouth.
  • Do not swallow the cream.
  • Do not apply to significantly irritated or broken skin unless advised by a healthcare professional.
  • Do not use in children under 2 years of age unless directed by a doctor.
  • Stop treatment and seek medical advice if significant irritation, worsening of the condition, or an allergic reaction develops.
  • If symptoms do not improve within the expected treatment period, medical evaluation is recommended because the condition may require a different diagnosis or treatment.

Possible Side Effects

Topical miconazole is generally well tolerated. Possible local adverse effects include:

  • Mild skin irritation
  • Burning or stinging
  • Itching
  • Redness
  • Local discomfort
  • Skin sensitivity or allergic contact dermatitis

WHO references report irritation, burning, maceration and allergic contact dermatitis as possible adverse effects.

If severe or persistent irritation occurs, discontinue use and consult a healthcare professional.

Important Considerations

Correct diagnosis:
Not every itchy or scaly rash is caused by a fungal infection. WHO guidance emphasizes establishing the correct diagnosis before treatment with topical antifungal preparations.

Scalp and nails:
Topical miconazole cream is generally not appropriate as sole treatment for fungal infections of the scalp or nails, where systemic or alternative treatment may be required.

Vaginal infections:
A topical skin cream should not automatically be substituted for a vaginal miconazole formulation. Products specifically formulated and labeled for vaginal use should be used for vulvovaginal candidiasis.

Drug Interactions

Because systemic absorption from intact skin is generally limited, clinically significant systemic interactions are uncommon with topical miconazole. However, patients should inform their healthcare professional about all medicines they use.

Particular caution may be appropriate in patients using warfarin, because miconazole products can potentially increase anticoagulant effects, especially with preparations or routes associated with greater systemic absorption.
