Medical Description: Carbetocin

Carbetocin is a long-acting synthetic analogue of the natural hormone oxytocin with uterotonic and anti-hemorrhagic activity. It acts by binding to oxytocin receptors on uterine smooth muscle, stimulating sustained uterine contractions that help increase uterine tone and reduce blood loss after childbirth. Because of its longer duration of action compared with oxytocin, a single dose can provide prolonged uterine contraction support.

Therapeutic Indications

  • Prevention of postpartum hemorrhage (PPH), especially hemorrhage related to uterine atony after delivery.
  • Commonly used after cesarean section and in selected vaginal deliveries according to institutional protocols and guideline recommendations.

Mechanism of Action
Carbetocin activates peripheral oxytocin receptors located in the myometrium (uterine muscle), producing:

  • Increased uterine tone
  • Increased frequency and strength of contractions
  • Reduced excessive postpartum bleeding

Pharmacologic Characteristics

  • Long-acting oxytocin receptor agonist
  • Longer half-life than oxytocin (approximately 4–10 times longer)
  • Usually administered as a single intravenous (IV) or intramuscular (IM) dose rather than prolonged infusion

Possible adverse effects may include:
