CHARTOREG PLUS 40 / 12.5 MG ( TELMISARTAN / HYDROCHLOROTHIAZIDE ) 28 TABLETS

CHARTOREG PLUS 40 / 12.5 MG

Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg — 28 Tablets
Angiotensin II Receptor Blocker (ARB) + Thiazide Diuretic | Antihypertensive

Medical Description

CHARTOREG PLUS 40/12.5 mg contains a fixed-dose combination of telmisartan 40 mg and hydrochlorothiazide 12.5 mg, two antihypertensive medicines with complementary mechanisms of action.

Telmisartan is an angiotensin II receptor blocker (ARB) that selectively blocks the AT₁ receptors activated by angiotensin II. This reduces angiotensin II-mediated vasoconstriction and promotes relaxation of blood vessels, helping to lower blood pressure.

Hydrochlorothiazide (HCTZ) is a thiazide diuretic that increases the renal excretion of sodium and water. This reduces fluid volume and contributes to lowering blood pressure.

The telmisartan + hydrochlorothiazide combination is an established antihypertensive combination; the 40/12.5 mg strength is also listed among available fixed-dose combinations in hypertension treatment resources.

Indication

CHARTOREG PLUS 40/12.5 mg is indicated for the treatment of hypertension (high blood pressure) in adults when combination therapy is clinically appropriate.

Combining an ARB with a thiazide diuretic provides blood-pressure reduction through two complementary mechanisms. Combination therapy is included among evidence-based approaches to pharmacological treatment of hypertension.

Lowering elevated blood pressure helps reduce the risk of cardiovascular complications associated with hypertension, particularly stroke and myocardial infarction.

How It Works

Telmisartan 40 mg

  • Blocks angiotensin II at AT₁ receptors.
  • Reduces vasoconstriction.
  • Promotes relaxation of blood vessels.
  • Helps lower systemic blood pressure.

Hydrochlorothiazide 12.5 mg

  • Acts on the kidneys as a thiazide diuretic.
  • Increases elimination of sodium and water through urine.
  • Helps reduce circulating fluid volume and blood pressure.

Dosage & Administration

  • CHARTOREG PLUS 40/12.5 mg should be taken exactly as prescribed by the treating physician.
  • Fixed-dose telmisartan/HCTZ combinations are generally administered once daily.
  • The 40/12.5 mg strength may be selected according to the patient's previous antihypertensive treatment, blood-pressure response, and clinical characteristics.
  • The U.S. prescribing information specifically identifies 40/12.5 mg as a starting combination strength in patients with certain hepatic or biliary conditions; other patients may require different strengths depending on their previous treatment.
  • It may be taken with or without food.
  • Take the tablet at approximately the same time each day.
  • Do not change the dose or discontinue treatment without medical advice.

Important Precautions

Tell your physician if you have:

  • Kidney disease or impaired kidney function
  • Liver disease
  • Dehydration or low blood pressure
  • Electrolyte abnormalities
  • Diabetes
  • Gout or elevated uric acid
  • Systemic lupus erythematosus
  • A history of allergic reactions to ARBs or sulfonamide-derived medicines

Hydrochlorothiazide can cause changes in sodium, potassium and other electrolytes, while telmisartan can increase serum potassium and affect kidney function in susceptible patients. Appropriate monitoring may therefore be required.

Patients should not use potassium supplements or potassium-containing salt substitutes without discussing them with their healthcare professional.

The combination of an ACE inhibitor with an ARB should generally be avoided because dual blockade of the renin-angiotensin system increases the risk of kidney problems, hyperkalemia and hypotension.

Pregnancy Warning

CHARTOREG PLUS should not be used during pregnancy.

Telmisartan affects the renin-angiotensin system and can cause serious fetal injury or death, particularly during the later stages of pregnancy. If pregnancy occurs while taking this medicine, contact your healthcare professional immediately.

Possible Side Effects

Possible adverse effects may include:

  • Dizziness or lightheadedness
  • Low blood pressure
  • Diarrhea
  • Increased urination
  • Changes in blood potassium or sodium levels
  • Changes in kidney function
  • Increased uric acid levels or worsening of gout
  • Changes in blood glucose levels

Hydrochlorothiazide can cause electrolyte disturbances and may affect glucose and uric acid metabolism, particularly in susceptible patients.

Seek prompt medical attention for severe dizziness/fainting, significant reduction in urine output, severe allergic symptoms, or swelling of the face, lips, tongue or throat.

Missed Dose

If you miss a dose, take it when you remember unless it is nearly time for your next scheduled dose. Do not take two doses at the same time to compensate for a missed dose.

Lifestyle Measures

Medication is an important component of hypertension management, but lifestyle measures can also contribute to blood-pressure control. These include:

  • Following a healthy, lower-sodium diet
  • Maintaining a healthy body weight
  • Regular physical activity
  • Avoiding tobacco
  • Limiting alcohol consumption

WHO recommends lifestyle modification alongside appropriate pharmacological treatment for adults with hypertension.

Storage

Keep CHARTOREG PLUS in its original packaging, protected from moisture and according to the storage conditions stated on the product packaging.

Patient Disclaimer

This information is provided for educational purposes only and does not replace the advice of a qualified physician or pharmacist. CHARTOREG PLUS 40/12.5 mg should be used only as prescribed. Do not start, stop, or change the dose without consulting your healthcare professional. Patients with kidney or liver disease, dehydration, electrolyte abnormalities, diabetes, gout, pregnancy or possible pregnancy, or those taking medicines that affect blood pressure, potassium levels, kidney function, or fluid balance should consult their healthcare professional before use. Regular blood-pressure monitoring and, when clinically indicated, monitoring of kidney function and electrolytes may be required.
