CARDIO - MEP 150 MG / 3 ML ( AMIODARONE ) 6 IV AMPOULES
CARDIO-MEP 150 mg/3 mL (Amiodarone) — IV Ampoules
Active ingredient: Amiodarone • Strength: 150 mg/3 mL • Dosage form: Intravenous injection
Medical Description
CARDIO-MEP 150 mg/3 mL contains amiodarone, a potent antiarrhythmic medicine used in the hospital management of selected serious cardiac rhythm disorders. It acts on the electrical activity of the heart to help suppress abnormal rhythms and stabilize cardiac function.
Amiodarone injection is particularly used for the treatment of recurrent ventricular fibrillation and hemodynamically unstable ventricular tachycardia that have not responded adequately to other treatments. Intravenous administration allows the medicine to be delivered rapidly when clinically necessary, under continuous medical supervision.
Each 3 mL ampoule contains 150 mg of amiodarone, according to the stated product strength. The pack contains six ampoules.
Indications and Potential Benefits
Amiodarone injection may be used for:
- Ventricular tachycardia (VT): Management of selected serious ventricular rhythm disturbances, particularly when other treatments have been ineffective.
- Ventricular fibrillation (VF): Treatment and prevention of recurrent episodes in patients who have not responded adequately to other therapy.
- Acute arrhythmia management: Stabilization of selected serious cardiac arrhythmias in appropriately monitored clinical settings.
- Patients unable to take oral therapy: Intravenous treatment may be considered when amiodarone is indicated but oral administration is not feasible.
Use for other arrhythmias depends on the clinical circumstances and the applicable local prescribing information.
Mechanism of Action
Amiodarone is classified primarily as a Class III antiarrhythmic agent under the Vaughan Williams classification. It blocks potassium channels, prolonging cardiac repolarization and the effective refractory period. It also has sodium-channel-blocking, calcium-channel-blocking, and noncompetitive antiadrenergic effects.
These combined actions slow abnormal electrical conduction, influence heart rate, and help suppress potentially dangerous cardiac arrhythmias.
Dosage and Administration
- Route: Intravenous administration by qualified healthcare professionals.
- Dose: Individualized according to the type and severity of the arrhythmia, the patient's clinical condition, and response to treatment.
- Administration method: Generally administered as a diluted intravenous infusion using an appropriate infusion pump and compatible diluent, according to the approved product instructions and clinical protocol.
- Monitoring: Continuous ECG and blood-pressure monitoring are required during acute intravenous treatment.
For certain life-threatening ventricular arrhythmias, official prescribing information describes an initial dose of 150 mg diluted in 100 mL of 5% dextrose and infused over 10 minutes, followed by a specified maintenance infusion regimen. This is an example of a clinical protocol, not a universal dosing instruction for every patient or product.
Do not administer the ampoule without verifying the product-specific instructions. Rapid administration or excessive infusion rates can cause severe hypotension, cardiovascular collapse, and serious organ injury.
Contraindications and Precautions
Intravenous amiodarone is contraindicated in certain patients, including those with:
- Cardiogenic shock.
- Marked sinus bradycardia.
- Second- or third-degree atrioventricular block without a functioning pacemaker.
- Known hypersensitivity to amiodarone or relevant formulation ingredients.
Use caution in patients with pre-existing thyroid or liver disease, pulmonary disorders, electrolyte disturbances, or impaired cardiac conduction. Pregnancy and breastfeeding require an individualized specialist assessment.
Possible Side Effects
Common or clinically important adverse effects include:
- Hypotension, particularly during intravenous infusion.
- Slow heart rate (bradycardia).
- Atrioventricular conduction disturbances.
- Nausea and other gastrointestinal symptoms.
- Infusion-site inflammation or phlebitis.
- Elevated liver enzymes or other hepatic abnormalities.
Serious adverse reactions may include:
- Severe bradycardia, heart block, or cardiac arrest.
- Worsening arrhythmias, including potentially dangerous ventricular arrhythmias.
- Severe hypotension or cardiogenic shock.
- Acute liver injury.
- Pulmonary toxicity, particularly with continued exposure.
Continuous clinical observation is essential so that adverse effects can be identified and managed promptly.
Drug Interactions
Amiodarone may interact with several medicines, including:
- Digoxin: May increase digoxin concentrations and toxicity risk.
- Warfarin: May enhance anticoagulant effects and increase bleeding risk.
- Beta-blockers and certain calcium-channel blockers: May increase the risk of bradycardia and atrioventricular block.
- Other QT-prolonging medicines and antiarrhythmics: May increase the risk of serious rhythm disturbances.
- Certain statins and other medicines metabolized through drug-interaction pathways: May require dose adjustment or closer monitoring.
A complete medication review is necessary before treatment whenever clinically feasible.
Monitoring During Treatment
Monitoring should include:
- Continuous ECG and heart-rate assessment.
- Frequent blood-pressure monitoring.
- Serum potassium and magnesium assessment, with correction of abnormalities.
- Liver-function assessment when indicated.
- Monitoring for infusion-site complications and signs of organ toxicity.
- Thyroid and pulmonary assessment when indicated by treatment duration, symptoms, or clinical circumstances.
Storage
Store according to the manufacturer's approved instructions on the package leaflet. Inspect ampoules for damage, discoloration, or visible particles before use. Do not use damaged ampoules or solutions that do not meet the product's specifications. Keep out of reach of children.
