Dapagliflozin — Medical Description

Dapagliflozin is an oral sodium-glucose cotransporter-2 (SGLT2) inhibitor used primarily in the management of type 2 diabetes mellitus, and also for the treatment of chronic heart failure and chronic kidney disease (CKD). Its cardiovascular and renal benefits can occur independently of its glucose-lowering effect.

Mechanism of Action

Dapagliflozin selectively inhibits SGLT2, a protein responsible for reabsorbing filtered glucose and sodium in the proximal renal tubules. Inhibition of SGLT2 increases urinary excretion of glucose and sodium, lowering blood glucose and producing mild osmotic diuresis and natriuresis. These effects contribute to reduced intravascular volume and favorable cardiovascular and renal effects.

Medical Uses

  • Type 2 Diabetes Mellitus: Used as an adjunct to diet and exercise to improve glycemic control, either alone when metformin is inappropriate or in combination with other antidiabetic medicines. It is authorized in the EU for adults and children 10 years and older.
  • Chronic Heart Failure: Used to reduce the risk of heart-failure-related clinical deterioration, including hospitalization, and cardiovascular outcomes in appropriate patients. Its benefits are observed regardless of whether the patient has type 2 diabetes.
  • Chronic Kidney Disease: Used to slow progression of CKD and reduce the risk of clinically important renal and cardiovascular outcomes in appropriate patients. Clinical evidence demonstrates benefit in patients with CKD both with and without type 2 diabetes.

Dosage

The commonly recommended dose for type 2 diabetes, chronic heart failure, and chronic kidney disease is 10 mg once daily, according to current European product information. The appropriate dose and suitability should be determined according to the indication, renal function, concomitant medicines, and local prescribing information.

  • Hypoglycemia, particularly when used with insulin or insulin secretagogues rather than when used alone.

Important Warnings & Precautions

Diabetic ketoacidosis (DKA) is an important, potentially serious adverse effect. It can occur even when blood glucose is not markedly elevated. Patients should seek immediate medical attention for symptoms such as nausea, vomiting, abdominal pain, excessive thirst, difficulty breathing, unusual fatigue or confusion.

Dapagliflozin may cause volume depletion and hypotension, particularly in older adults, patients with impaired renal function, those with low blood pressure, or patients receiving diuretics. Renal function and volume status should be assessed when clinically appropriate.

Patients should also be monitored for serious genitourinary infections, including pyelonephritis, urosepsis, severe genital infections and the rare but serious Fournier's gangrene.

When dapagliflozin is combined with insulin or insulin secretagogues, the risk of hypoglycemia may increase and adjustment of the concomitant medication may be required.

Pregnancy & Breastfeeding

Dapagliflozin is not recommended during pregnancy, particularly during the second and third trimesters, because of potential fetal renal-development concerns. It is also not recommended during breastfeeding according to current U.S. prescribing information.
