Overview

Gemfibrozil is an oral lipid-lowering medication belonging to the fibric acid derivative (fibrate) class. It is primarily indicated for the treatment of hypertriglyceridemia and certain types of mixed dyslipidemia when lifestyle modifications alone are insufficient. Gemfibrozil works by activating peroxisome proliferator-activated receptor alpha (PPAR-α), resulting in enhanced breakdown of triglyceride-rich lipoproteins, reduced hepatic production of very-low-density lipoprotein (VLDL), and increased high-density lipoprotein (HDL) cholesterol.

Mechanism of Action

Promotes fatty acid oxidation.
Increases circulating HDL cholesterol.

These effects typically reduce triglyceride levels by 30–60%, with modest increases in HDL cholesterol.

Adult Use

Indications

Gemfibrozil is indicated for adults with:

Severe hypertriglyceridemia (especially when triglycerides are high enough to increase the risk of pancreatitis).
Mixed dyslipidemia in selected patients who have not achieved adequate lipid control with diet and lifestyle modification.
Certain patients at increased cardiovascular risk when clinically appropriate.

Usual Adult Dose

600 mg orally twice daily, taken 30 minutes before the morning and evening meals.
Patients should continue a heart-healthy diet, regular exercise, and weight management during therapy.

Pediatric Use

Gemfibrozil is not routinely recommended for children. Safety and effectiveness in pediatric patients have not been well established, and its use should be limited to situations where a specialist determines that the potential benefits outweigh the risks.

Contraindications

Do not use gemfibrozil in patients with:

Hypersensitivity to gemfibrozil.
Severe hepatic impairment.
Severe renal impairment.
Pre-existing gallbladder disease.
Primary biliary cirrhosis.
Concomitant use with certain medications such as repaglinide, dasabuvir, or simvastatin, due to the risk of serious adverse interactions.

Warnings and Precautions

Gemfibrozil may increase the risk of myopathy and rhabdomyolysis, particularly when used with statins.
Monitor liver function tests periodically during treatment.
Monitor lipid levels to assess therapeutic response.
Use cautiously in patients with impaired kidney function.
Discontinue treatment if unexplained muscle pain, tenderness, or weakness occurs, particularly if accompanied by fever or dark-colored urine.
Patients should maintain dietary modifications and other lifestyle measures throughout treatment.

Drug Interactions

Gemfibrozil may interact with:

Statins (especially simvastatin) – increased risk of muscle injury.
Warfarin – may enhance anticoagulant effects; INR monitoring is recommended.
Repaglinide – contraindicated due to severe hypoglycemia risk.
Colchicine – increased risk of muscle toxicity.
Certain CYP2C8 substrate medications requiring dose adjustment or avoidance.

Pregnancy and Lactation

Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.
It is not generally recommended during breastfeeding, as safety data are limited. A healthcare provider should determine the most appropriate therapy.
