Insulin Detemir (Levemir®) – Clinical Information
Insulin detemir is a long-acting basal insulin indicated for the management of Type 1 and Type 2 diabetes mellitus. It provides sustained blood glucose control for up to 24 hours and may be used alone or in combination with rapid-acting insulin and/or oral antidiabetic medications.
Mechanism of Action
Insulin detemir helps regulate blood glucose by facilitating cellular glucose uptake and reducing hepatic glucose production. Its prolonged duration of action supports stable basal insulin coverage throughout the day.
Indications
- Type 1 diabetes mellitus (adults and children ≥2 years)
- Type 2 diabetes mellitus (adults; pediatric use as determined by a healthcare provider)
Administration
- Administered by subcutaneous injection into the abdomen, thigh, or upper arm.
- May be given once or twice daily according to physician recommendations.
- Injection sites should be rotated regularly to minimize injection-site reactions and lipodystrophy.
- Do not mix insulin detemir with other insulin products.
- The solution should be clear and colorless before use.
Important Warnings and Precautions
- Hypoglycemia is the most common and potentially serious adverse effect.
- Regular blood glucose monitoring is essential.
- Never share insulin pens, cartridges, or needles with others.
- Use with caution in patients with:
- Kidney impairment
- Liver impairment
- Heart failure
- Low potassium levels (hypokalemia)
- Not indicated for the treatment of diabetic ketoacidosis (DKA).
- Alcohol consumption may affect blood glucose control.
- Certain medications may alter insulin requirements or increase the risk of hypoglycemia.
Signs of Hypoglycemia (Low Blood Sugar)
Patients should be educated to recognize symptoms such as:
- Sweating
- Shakiness
- Hunger
- Dizziness
- Confusion
- Blurred vision
- Rapid heartbeat
Prompt treatment with a fast-acting carbohydrate source is recommended.
Signs of Hyperglycemia (High Blood Sugar)
Symptoms may include:
- Excessive thirst
- Frequent urination
- Fatigue
- Blurred vision
- Dry mouth
- Nausea or vomiting
Medical advice should be sought if these symptoms occur.
Serious Adverse Reactions
Seek immediate medical attention if any of the following occur:
- Severe allergic reactions (anaphylaxis)
- Swelling of the face, lips, tongue, or throat
- Difficulty breathing
- Severe hypoglycemia with loss of consciousness or seizures
- Signs of fluid retention or worsening heart failure
Common Side Effects
- Injection-site reactions (redness, itching, swelling, or pain)
- Weight gain
- Peripheral edema (fluid retention)
- Mild skin reactions at injection sites
Storage
- Unopened pens/vials: Refrigerate; may be kept at room temperature for up to 42 days. Do not freeze.
- Opened vials: Refrigerate or store at room temperature; use within 42 days.
- Opened pens: Store at room temperature and discard after 42 days.
- Protect from excessive heat and direct light.
Patient Counseling Points
- Follow the prescribed dosing schedule carefully.
- Monitor blood glucose regularly.
- Carry a source of fast-acting sugar and medical identification indicating diabetes.
- Consult a healthcare provider before starting new medications, supplements, or herbal products.
- Maintain regular follow-up appointments to assess treatment response and safety.
Patient Disclaimer:
Insulin detemir should only be used under the supervision of a qualified healthcare professional. Dosage and treatment plans must be individualized based on blood glucose monitoring, clinical response, and patient-specific factors. Patients should not adjust their insulin dose without consulting their healthcare provider.
