TIBOLONE TABLETS – Medical Description

Overview

Tibolone is a synthetic steroid indicated for the management of estrogen deficiency symptoms in postmenopausal women. After oral administration, tibolone is rapidly metabolized into active metabolites that exhibit estrogenic, progestogenic, and androgenic activities in a tissue-selective manner. This unique pharmacological profile helps relieve menopausal symptoms while minimizing stimulation of the endometrium when used appropriately.

Tibolone is primarily prescribed for women who are at least 12 months after their final natural menstrual period. It is also approved in many countries for the prevention of postmenopausal osteoporosis in women at increased risk of fracture when other therapies are unsuitable or not tolerated.

Adult Use

Indications

Usual Adult Dose

• 2.5 mg orally once daily, taken at approximately the same time each day.
• Tablets may be taken with or without food.
• Treatment should use the lowest effective dose for the shortest duration consistent with treatment goals.

Pediatric Use

Tibolone is not indicated for children or adolescents. Its safety and effectiveness have not been established in pediatric patients.

Contraindications

Do not use tibolone in patients with:

• Known or suspected breast cancer.
• Known or suspected estrogen-dependent malignancy (e.g., endometrial cancer).
• Undiagnosed vaginal bleeding.
• Untreated endometrial hyperplasia.
• Previous or active venous thromboembolism (deep vein thrombosis or pulmonary embolism).
• Active or recent arterial thromboembolic disease (e.g., stroke or myocardial infarction).
• Severe liver disease or impaired liver function.
• Known hypersensitivity to tibolone or any tablet ingredient.
• Pregnancy or breastfeeding.

Warnings and Precautions

• Report any unexpected vaginal bleeding, particularly after the first few months of treatment.
• Tibolone may increase the risk of stroke, particularly in older women.

Drug Interactions

Tibolone may interact with:

• Enzyme-inducing drugs such as carbamazepine, phenytoin, rifampicin, and phenobarbital, which may reduce tibolone effectiveness.
• Herbal preparations containing St. John's wort, which may decrease tibolone concentrations.

Pregnancy and Lactation

• Contraindicated during pregnancy.
• Do not use while breastfeeding.
• If pregnancy occurs during treatment, tibolone should be discontinued immediately.
