Esomeprazole is a proton pump inhibitor (PPI) and the S-enantiomer of omeprazole, specifically designed to provide potent, long-lasting suppression of gastric acid. 

Medical Description
Mechanism of Action: It suppresses gastric acid secretion by irreversibly binding to and inhibiting the H+/K+ -ATPase enzyme system (the "proton pump") in the parietal cells of the stomach lining. This blocks the final step of acid production, significantly raising gastric pH levels for over 24 hours.
 

Pharmacokinetics:

  • Absorption: Rapidly absorbed after oral administration, typically reaching peak plasma levels within 1–2 hours. It is acid-labile and is formulated as delayed-release pellets to ensure it survives stomach acid and is absorbed in the small intestine.
  • Metabolism: Extensively metabolized in the liver by CYP2C19 and CYP3A4 enzymes into inactive metabolites.
  • Elimination: Approximately 80% is excreted as metabolites in the urine and 20% in feces.
  • Indications: Approved for treating GERD (including erosive esophagitis healing and maintenance), Zollinger-Ellison Syndrome, H. pylori eradication (in combination with antibiotics), and reducing the risk of NSAID-associated gastric ulcers.

  • Allergic Reactions: Do not use if you have a known hypersensitivity to esomeprazole or other substituted benzimidazoles (like lansoprazole or omeprazole). Immediate medical attention is required for signs of anaphylaxis, such as swelling of the face/throat or difficulty breathing.
  • Long-Term Risks: Extended use (typically over 1 year) may increase the risk of bone fractures (hip, wrist, or spine), Vitamin B12 deficiency, and severe magnesium depletion (hypomagnesemia), which can lead to seizures or heart arrhythmias.
  • Serious Infections: PPI use is associated with an increased risk of Clostridioides difficile-associated diarrhea (CDAD). Seek medical help for severe, watery, or bloody diarrhea that does not improve.
  • Masking Symptoms: Relief of symptoms does not rule out the presence of gastric malignancy (stomach cancer). Consult a doctor if you experience unexplained weight loss, persistent vomiting, or difficulty swallowing.
  • Drug Interactions: Can significantly interact with Clopidogrel (reducing its effectiveness), Warfarin (increasing bleeding risk), Digoxin, and certain HIV medications (like atazanavir). 


Usage Instructions

  • Timing: For maximum benefit, take at least one hour before a meal, preferably in the morning.
  • Administration: Swallow capsules whole. If you have difficulty swallowing, the capsule may be opened and the intact granules mixed with one tablespoon of cool applesauce. Swallow the mixture immediately without chewing or crushing the granules.
