MONICURE NH ( MICONAZOLE NITRATE + NYSTATIN + NEOMYCIN SULFATE + HYDROCORTISONE ) 6 VAGINAL SUPPOSITORIES

MONICURE NH Vaginal Suppositories

Miconazole Nitrate 100 mg + Nystatin 100,000 IU + Neomycin Sulfate 50 mg + Hydrocortisone Acetate 5 mg

Medical Description

MONICURE NH Vaginal Suppositories are a combination vaginal preparation containing two antifungal agents, an antibacterial antibiotic, and a corticosteroid. The combination is designed to provide broad local antimicrobial activity while helping reduce vaginal and vulvovaginal inflammation, itching, redness and irritation associated with susceptible mixed or complicated vaginal infections.

Composition

Each vaginal suppository contains:

  • Miconazole Nitrate 100 mg – an imidazole antifungal that inhibits fungal growth and is commonly used for vaginal candidiasis.
  • Nystatin 100,000 IU – a polyene antifungal with activity against Candida species. WHO includes intravaginal nystatin among treatment options for vulvovaginal candidiasis.
  • Neomycin Sulfate 50 mg – an aminoglycoside antibiotic providing antibacterial activity against susceptible organisms.
  • Hydrocortisone Acetate 5 mg – a corticosteroid intended to reduce local inflammatory manifestations such as redness, swelling and itching. Hydrocortisone has anti-inflammatory and antipruritic effects.
  • Excipients: Hard fat suppository base.

Indications & Potential Benefits

MONICURE NH may be prescribed for the local treatment of susceptible vaginal and vulvovaginal infections, particularly when fungal infection is accompanied by significant inflammation or when mixed microbial involvement is suspected.

Its four-component formulation provides:

1. Antifungal activity
Miconazole and nystatin target fungal organisms, particularly Candida, which is a common cause of vulvovaginal candidiasis. Vaginal candidiasis may present with itching, soreness, redness, abnormal discharge, burning and discomfort.

2. Antibacterial activity
Neomycin provides local antibacterial activity against susceptible bacteria and may be useful when bacterial involvement is part of the clinical picture.

3. Anti-inflammatory and antipruritic action
Hydrocortisone helps decrease inflammatory manifestations such as redness, swelling and itching, potentially providing symptomatic relief while the underlying infection is treated.

Mechanism of Action

Miconazole:
Miconazole is an imidazole antifungal that interferes with fungal cell-membrane formation, thereby inhibiting fungal growth.

Nystatin:
Nystatin is a polyene antifungal that acts against susceptible fungi, including Candida. WHO recognizes intravaginal nystatin as a treatment option for vulvovaginal candidiasis.

Neomycin:
Neomycin is an aminoglycoside antibiotic that interferes with bacterial protein synthesis, resulting in inhibition of susceptible bacterial growth.

Hydrocortisone:
Hydrocortisone is a corticosteroid that reduces inflammatory responses, helping relieve itching, redness and swelling.

Directions for Use

Use only according to the prescribing physician's instructions and the product leaflet, as the appropriate duration depends on the diagnosis and clinical circumstances.

For vaginal suppositories:

  1. Wash hands before use.
  2. Remove the suppository from its packaging.
  3. Insert the suppository deep into the vagina, preferably at bedtime.
  4. Wash hands after administration.
  5. Complete the prescribed course even if symptoms improve earlier, unless advised otherwise by the healthcare professional.

For vaginal symptoms, accurate diagnosis is important because itching, burning, discharge and irritation are not specific to candidiasis and may have bacterial, parasitic, dermatological or other causes.

Important Warnings & Precautions

  • For vaginal use only; do not swallow.
  • Do not use if you have a known allergy to miconazole, nystatin, neomycin, hydrocortisone, or any component of the formulation.
  • Prolonged or inappropriate use of corticosteroid-containing preparations should be avoided unless specifically prescribed.
  • Stop treatment and seek medical advice if severe irritation, rash, swelling or an allergic reaction develops.
  • Consult a physician if symptoms persist, worsen, or recur frequently after treatment.
  • Recurrent or complicated vulvovaginal candidiasis may require laboratory evaluation and identification of the causative organism.
  • Pregnant or breastfeeding women should consult their physician before use, particularly because this is a combination preparation containing a corticosteroid and antibiotic.
  • Avoid self-treatment when symptoms are accompanied by fever, pelvic/lower abdominal pain, foul-smelling discharge, bleeding, or significant urinary symptoms; medical evaluation is recommended.

Possible Side Effects

Local adverse effects may include:

  • Vaginal burning or irritation
  • Itching or discomfort
  • Redness
  • Local hypersensitivity or allergic reactions
  • Irritation related to any of the active ingredients

Topical vaginal antifungal preparations can cause local burning or irritation in some patients.

Important Clinical Note

MONICURE NH should not be considered a routine treatment for every case of vaginal discharge or itching. The CDC emphasizes that symptoms of vulvovaginal candidiasis are not specific, and appropriate clinical evaluation may be necessary to distinguish candidiasis from bacterial vaginosis, trichomoniasis and other causes of vaginitis.

WHO also recognizes miconazole and nystatin as intravaginal treatment options for vulvovaginal candidiasis, with treatment selection depending on the patient and clinical circumstances.
