Filgrastim is a man-made version of Granulocyte Colony-Stimulating Factor (G-CSF), a protein that stimulates bone marrow to produce and release neutrophils (infection-fighting white blood cells). 
  • Neutropenia Management: Prevents or reduces the duration of low white blood cell counts caused by myelosuppressive chemotherapy or radiation.
  • Stem Cell Collection: Mobilizes stem cells into the blood for collection before transplants.
  • Radiation Sickness: Used to improve survival in patients exposed to life-threatening doses of radiation.
  • Bone Marrow Recovery: Assists recovery following a bone marrow transplant. 

Administration Guidelines
  • Timing: The first dose must be given at least 24 hours after chemotherapy and should not be given within 24 hours before the next chemo cycle.
  • Site Rotation: Common injection sites include the abdomen (avoiding 2 inches around the navel), thighs, or upper arms. Rotating sites daily helps prevent skin irritation or scar tissue.
  • Preparation: Remove the syringe from the refrigerator 30 minutes before use to reach room temperature, which reduces stinging. Do not shake the medication as it can damage the protein. 

Critical Side Effects & Precautions
  • Bone Pain: The most common side effect; it can often be managed with antihistamines like loratadine (Claritin) or analgesics, but consult your doctor first.
  • Latex Allergy: Many prefilled syringe needle caps contain natural rubber latex. Patients with latex allergies should alert their care team.
  • Serious Warnings: Seek immediate medical care for:
  • Splenic Rupture: Indicated by pain in the upper left stomach or left shoulder.
  • Lung Problems: Shortness of breath, fever, or coughing up blood (ARDS).
  • Capillary Leak Syndrome: Sudden swelling, low blood pressure, or decreased urination. 

Storage & Handling
  • Temperature: Store in the refrigerator between 2°C – 8°C (36°F – 46°F).
  • Light Protection: Keep syringes in the original carton to protect them from light.
  • Room Temp Limit: Most brands can stay at room temperature for up to 24 hours; if left out longer, they must be discarded.
