Description

Zolmitriptan is a selective serotonin receptor agonist (5-HT1B/5-HT1D receptor agonist) belonging to the triptan class of medications. It is primarily indicated for the acute treatment of migraine attacks with or without aura in adults. Zolmitriptan is intended to relieve an active migraine episode and is not used for migraine prevention.

Mechanism of Action

Zolmitriptan acts by stimulating serotonin (5-HT1B/1D) receptors in cranial blood vessels and trigeminal sensory nerves. This action results in:

  • Constriction of dilated intracranial blood vessels
  • Inhibition of inflammatory neuropeptide release (including CGRP and substance P)
  • Reduction of pain transmission through trigeminal pathways

These effects collectively help reduce migraine symptoms such as headache pain, nausea, photophobia (light sensitivity), and phonophobia (sound sensitivity).

Indications

Zolmitriptan is commonly used for:

  • Acute migraine attacks with or without aura
  • Selected cases of acute cluster headache (primarily nasal formulations, depending on clinical judgment)

It should not be used for:

  • Migraine prevention
  • Non-migraine headaches
  • General pain management

Available Dosage Forms

Zolmitriptan may be available as:

  • Oral tablets
  • Orally disintegrating tablets (ODT)
  • Nasal spray

Adult Dosage (Acute Migraine)

Oral tablets / orally disintegrating tablets:

  • Initial dose: 2.5 mg orally once
  • If symptoms recur: may repeat after at least 2 hours
  • Maximum single dose: 5 mg
  • Maximum daily dose: 10 mg within 24 hours

Nasal spray:

  • Initial dose: 2.5 mg or 5 mg intranasally
  • May repeat after at least 2 hours
  • Maximum daily dose: 10 mg within 24 hours

Common Adverse Effects

Potential side effects include:

  • Dizziness
  • Drowsiness
  • Dry mouth
  • Nausea
  • Weakness or fatigue
  • Tingling sensations (paresthesia)
  • Chest or throat tightness

Serious but less common adverse effects include cardiovascular events, serotonin syndrome, and significant blood pressure elevations.

Contraindications and Precautions

Zolmitriptan should generally be avoided in patients with:

  • Coronary artery disease
  • History of myocardial infarction
  • Stroke or transient ischemic attack
  • Uncontrolled hypertension
  • Severe peripheral vascular disease
  • Hemiplegic or basilar migraine

Caution is advised when used with SSRIs, SNRIs, MAO inhibitors, ergot derivatives, or other triptans because of interaction risks.
