Properties:
Famotak is a thiazide derivative of histamine.
It is a histamine H2 receptor antagonist that inhibits gastric acid secretion elicited by histamine or by gastrin.
Famotak inhibits basal (fasting), nocturnal acid secretion & that stimulated by food, fundic distention & various pharmacological agents.
Both the acid concentration & volume of gastric secretion are suppressed by Famotak, while changes in pepsin secretion are proportional to volume output.
The duration of inhibition of secretion after doses of 20 & 40 mg is usually 10 - 12 hours respectively.
Famotak is readily absorbed from the gastro-intestinal tract.
Bioavailability of oral doses is 40 - 45 %, this may be increased slightly by food or decreased slightly by antacids.
After oral doses, peak plasma levels are achieved within 1 - 3 hours.
Peak plasma level is not altered by chronic administration & elimination half life is 2.5 - 3.5 hours. It is eliminated largely unchanged by the renal route (65 - 70 %) & the remainder by metabolic routes. Famotak has no androgenic action. It does not affect prolactin levels, or sex hormones.

Indications:
Short term treatment of active gastric & duodenal ulcer.
Prevention of duodenal ulcer recurrence in selected patients.
Gastric hypersecretory conditions: in treatment of pathological gastric hyper-secretion associated with Zollinger-Ellison syndrome, systemic mastocytosis & multiple endocrine adenomas.
Symptomatic treatment of Gastro- Esophageal Reflux disease
Treatment of upper gastro-intestinal bleeding secondary to gastric ulcer, duodenal ulcer, or hemorrhage gastritis.
With NSAIDs & in minor digestive complains (eg. Chronic episodic dyspepsia).
Dosage :
For duodenal ulcer treatment : 40 mg once daily at bedtime or 20mg twice daily.
For prophylaxis of recurrent duodenal ulcer : 20 mg once daily at bed time
For treatment of benign gastric ulcer : 40 mg once daily at bedtime.
For treatment of gastro-esophageal reflux disease: 20 mg twice daily
Ulcerative esophagitis: 20 or 40 mg twice daily.
With NSAIDs & in minor digestive complains (eg. Chronic episodic dyspepsia) : 20 or 40 mg daily.
Pathological gastric hyper secretion: associated with Zollinger-Ellison syndrome, systemic mastocytosis & multiple endocrine adenomas : 20 mg every 6 hours, the dosage being adjusted as needed & therapy continued for as long as clinically indicated.
