Description

Tamsulosin is a selective alpha-1 adrenergic receptor blocker primarily used to treat the lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH). It selectively blocks α1A receptors located in the smooth muscle of the prostate, bladder neck, and prostatic urethra, resulting in relaxation of these muscles and improved urinary flow with minimal effects on systemic blood pressure compared with non-selective alpha blockers. It improves symptoms but does not reduce prostate size or halt disease progression.

Mechanism of Action

Tamsulosin selectively antagonizes postsynaptic α1A-adrenergic receptors found predominantly in the:

• Prostate
• Bladder neck
• Prostatic urethra

Relaxation of these smooth muscles leads to:

• Improved urine flow
• Reduced bladder outlet obstruction
• Decreased urinary urgency and frequency
• Better bladder emptying

Because of its receptor selectivity, tamsulosin generally produces fewer cardiovascular adverse effects than non-selective alpha blockers.

Indications

In selected patients, clinicians may prescribe tamsulosin for:

• Facilitation of ureteric stone passage (medical expulsive therapy)
• Functional bladder outlet obstruction
• Neurogenic lower urinary tract symptoms (selected cases)

These uses depend on clinical judgment and current guideline recommendations.

Recommended Dosage

Adults

Initial dose

• 0.4 mg orally once daily

Take approximately 30 minutes after the same meal each day.

If response is inadequate after 2–4 weeks, the dose may be increased to:

• 0.8 mg once daily

Capsules should be swallowed whole and must not be crushed, chewed, or opened.

Administration

• Take after the same meal each day.
• Swallow capsules whole with water.
• Maintain adequate hydration.
• If therapy is interrupted for several days, treatment is usually restarted at 0.4 mg daily.

Common Side Effects

Most adverse effects are mild and transient.

Common adverse reactions include:

• Dizziness
• Orthostatic hypotension
• Headache
• Fatigue
• Nasal congestion or runny nose
• Abnormal ejaculation (including decreased or retrograde ejaculation)
• Back pain

Serious Adverse Effects

Although uncommon, patients should seek immediate medical attention if they experience:

• Syncope (fainting)
• Severe hypotension
• Allergic reactions (rash, facial swelling, difficulty breathing)
• Priapism (painful erection lasting >4 hours)
• Intraoperative Floppy Iris Syndrome (IFIS) during cataract or glaucoma surgery

Patients should inform their ophthalmologist if they are taking or have recently taken tamsulosin before eye surgery.

Contraindications

Tamsulosin should not be used in patients with:

• Known hypersensitivity to tamsulosin or any formulation component
• Previous severe allergic reaction to the drug

Use with caution in patients with:

• Symptomatic hypotension
• Severe hepatic impairment
• Severe renal impairment

Drug Interactions

Clinically important interactions may occur with:

• Other alpha-blockers
• Antihypertensive medications
• PDE-5 inhibitors (e.g., sildenafil, tadalafil)
• Strong CYP3A4 inhibitors (e.g., ketoconazole)
• Strong CYP2D6 inhibitors (e.g., paroxetine)
• Certain macrolide antibiotics
• Ritonavir-containing regimens

These combinations may increase the risk of hypotension or alter tamsulosin exposure.

Use in Special Populations

Elderly

Tamsulosin is commonly prescribed in older men and is generally well tolerated, though the risk of dizziness and falls should be considered.

Women

Not indicated for routine use in women.

Pediatric Patients

Safety and effectiveness have not been established; tamsulosin is not routinely recommended for pediatric use.
